How to Use an Amsler Grid to Catch Vision Changes Early
One minute a day with a simple grid can catch wet macular degeneration weeks before you would otherwise notice — and those weeks matter.
The Amsler grid is a square filled with evenly spaced lines and a dot in the center. It is nearly a century old, costs nothing, and remains one of the best home tools for detecting changes in the macula.
Why It Works
Your brain is remarkably good at filling in gaps, and your two eyes cover for each other. That is why patients with a growing blind spot in one eye frequently notice nothing for months. Testing one eye at a time against a regular pattern strips away that compensation and reveals distortion immediately.
How to Test
- Wear your usual reading glasses.
- Hold the grid about 14 inches away in good, even light.
- Cover one eye completely with your palm.
- Stare directly at the center dot with the uncovered eye and keep your gaze there.
- Without moving your gaze, notice the rest of the grid: are all lines straight, all squares equal, all corners visible?
- Repeat with the other eye.
Do this once a day if you have wet AMD or are at high risk, and at least weekly for intermediate dry AMD, epiretinal membrane, or diabetes.
What to Look For
- Lines that appear wavy, bent, or broken
- Squares that look larger, smaller, or misshapen
- Blurred, gray, or missing areas anywhere on the grid
- Corners you cannot see while fixating on the center
- Any change compared with yesterday
What to Do If Something Changes
Call us. New or worsening distortion can indicate conversion from dry to wet AMD, new fluid in a treated eye, or a developing macular problem. Wet AMD treated within days does far better than wet AMD treated within months. We keep appointment slots available for exactly this situation.
Do not talk yourself out of calling because "it is probably nothing." We would much rather examine a normal retina than miss a treatable one.
Practical Tips
- Tape the grid to the refrigerator or bathroom mirror so testing becomes routine
- Test at the same time of day in the same lighting
- Note the date and anything you see in a small notebook and bring it to visits
- Free printable grids are widely available, and smartphone apps can send reminders
What It Does Not Replace
The Amsler grid detects central changes only. It cannot find peripheral tears, detachments, or early diabetic damage. Keep your scheduled dilated exams and imaging appointments regardless of how the grid looks.
